Having problems with fillable PDF forms?

To download, complete and submit PDF forms you will need Adobe Reader installed on your computer. Download Adobe Reader for free.

When you download a PDF form, the default settings of your web browser may cause the PDF file to automatically open in the browser rather than in Adobe Reader. Most browsers use a different PDF viewer by default that doesn't support advanced features of fillable PDF forms, and you may not be able to complete all fields or submit the form.

  1. Make sure Acrobat Reader is on your computer.
  2. 右键单击PDF链接.
  3. Choose the "Save As" option (the exact option name will vary, depending on your browser).
  4. Save the document to your desktop or a folder where you can easily find it.
  5. Open Adobe Reader and use the file menu to open the PDF.

Student Financial Petition

This form is used by students wishing to petition charges for a course that they are no longer enrolled in.

Authorization for Fee Reduction

Employees, Employee Dependents & 老年人

This form is used to request and authorize fee reduction for faculty, 工作人员, 老年人, as well as dependents of faculty, 和工作人员.

  2. Select "Complete New Form"
  3. A notification will be sent to the Budget Custodian/ Supervisor that you indicated on the form (be sure to check the spelling of your supervisor’s email)

Request for Student or Borrower’s Taxpayer Identification Number and Certification

Northwest Missouri State University must obtain a student’s correct identifying number to file Form 1098-T, 学费的声明, with the IRS and to furnish a statement to you. This will be your Social Security Number (SSN) or, if you are not eligible to obtain an SSN, your individual taxpayer identification number (ITIN).  Form 1098-T contains information about qualified tuition and related expenses to help determine whether you, or the person who can claim you as a dependent, may take either the tuition and fees deduction or claim an education credit to reduce federal income tax.  For more information, see 国税局酒吧. 970, Tax Benefits for Higher Education.

Change Fund Request Form

This form is used by organizations or departments to request cash or startup money to have change for events, 销售, 筹款活动, 等.

This form is sent to customers for services or merchandise. One copy is sent to the customer and one is sent to Cashiering.
