Authorization for Fee Reduction

Employees, Employee Dependents & 老年人

This form is used to request and authorize fee reduction for faculty, 工作人员, 老年人, as well as dependents of faculty, 和工作人员.

2. Select "Complete New Form"
3. A notification will be sent to the Budget Custodian/ Supervisor that you indicated on the form (be sure to check the spelling of your supervisor’s email)

退休人员 & 退休人员家属

• For a retiree or retiree dependent, the Retiree Authorization for Fee Reduction form will need to be completed and returned to the Office of Human Resources.

• If fee reduction is based on a partnership, complete the Partnership Authorization for Fee Reduction form . The form will need to be completed and emailed to sfreeman@dctdsj.com.
  

All forms must be completed by the first day of the semester.

Request for Student or Borrower’s Taxpayer Identification Number and Certification

Northwest Missouri State University must obtain a student’s correct identifying number to file Form 1098-T, 学费的声明, with the IRS and to furnish a statement to you. This will be your Social Security Number (SSN) or, if you are not eligible to obtain an SSN, your individual taxpayer identification number (ITIN).  Form 1098-T contains information about qualified tuition and related expenses to help determine whether you, or the person who can claim you as a dependent, may take either the tuition and fees deduction or claim an education credit to reduce federal income tax.  For more information, see 国税局酒吧. 970, Tax Benefits for Higher Education.

Change Fund Request Form

This form is used by organizations or departments to request cash or startup money to have change for events, 销售, 筹款活动, 等.
